Photo Release Terms and Conditions:
ACKNOWLEDGEMENT
I hereby authorize Tracy Spaans Photography & Design to publish photographs taken of me, and my name, and likeness, for use in Tracy Spaans Photography & Design’s print, online and video-based marketing materials, as well as other publications.
I hereby release and hold harmless Tracy Spaans Photography & Design from any reasonable expectation of privacy or confidentiality for myself and/or my family members including any minor child and/or children listed below associated with the images specified above.
I further acknowledge that participation is voluntary and that neither I and/or the minor child or children will receive financial compensation or royalties.
I hereby agree, consent, and authorize Tracy Spaans of Tracy Spaans Photography & Design to use my likeness, and/or my minor child’s likeness in any photograph, video or other digital media taken or to be taken during a scheduled session or sporting event, or team photos, in any and all of its publications, including print or web-based publications.
I irrevocably authorize Tracy Spaans to copy, edit, enhance, crop, or otherwise alter any photo for use in their publication. I also waive the rights for approval or inspection of any photos.
I understand and agree that all photos are the property of Tracy Spaans Photography & Design.
I agree to release and forever discharge Tracy Spaans Photography & Design and its affiliates, successors and assigns, officers, employees, representatives, partners, agents and anyone claiming through them, in their individual and/or corporate capacities from any and all claims, liabilities, obligations, promises, agreements, disputes, demands, damages, causes of action of any nature or kind, known or unknown, which I, and anyone claiming on behalf of me, may have or claim to have against Releasee in connection with this Release.
I have carefully read and fully understand all the provisions of this photo release form and am freely knowingly and voluntarily signing on behalf of all listed.
